Applying for
Services
A child support case is automatically
opened when the child is on public assistance or the custodial parent
fills out an application for child support.
To apply for child support
enforcement services, the applicant must complete an Application for
Services and return it to the appropriate local office. Click
here to print an application for services. Applications can also
be requested by mail or in person at a local office. A IV-D
application will be provided on the day requested when the request is
in person, or provided within five working days if requested by phone
or mail.
The application can be completed in
the office or mailed to the office when it is complete. The
application form provides information on the following topics:
- The assignment of rights to
support payments;
- Available services;
- The individual's rights and
responsibilities;
- Fees, cost-recovery, and
distribution policies;
- The requirements for good cause
exemptions in public assistance cases; and
- The lack of an attorney/client
relationship.
All applications are logged into the
child support office upon receipt, the application is stamped with the
date it was submitted, and a case record is established. Within 20
days of receiving an application for services, a child support
specialist will review the case and contact the applicant to verify
information and solicit further information to assist in location
efforts. It is important for custodial parents to cooperate in the
child support process and provide the caseworker with all required
information.
Clients not receiving public
assistance will be required to pay a $20 application fee, which must
be submitted with the application for services.
